This is a two physician practice seeking to add an advanced practitioner 
to busy practice.

The advanced practitioner provides assessment, consultation, and 
intervention for patients and education for staff and families. They 
work collaboratively with the PCP; diagnose illness and physical 
condition and perform therapeutic and corrective measures.

Assesses the physical and psychosocial status of patients by interview, 
health history, physical exam and diagnostic tests.

initiatives. Carries out responsibilities with minimal need for 
supervision and guidance within the scope of practice.

- Demonstrates excellence in the completion of patient history and 
physical exams and assessments appropriate to patient population.

- Demonstrates proficiency in dictation of the history and physical and 
other notes on patient status.

- Coordinates/collaborates with clinical team to ensure development of 
appropriate treatment plans.

- Conducts patient visits and writes progress notes.

- Maintains regular communication with patients, families, and primary 
care physicians as appropriate regarding progress and treatment plans


Connecticut Licensed Nurse Practitioner

1-3 Years experience with outpatient adult medicine.
