NURSE PRACTITIONER **
Brooklyn, NY * Full Time, Day and Evening Shifts and Rotating Saturdays *

MSO of Kings County, LLC, founded in 2010, is an expanding for-profit 
organization that provides a diverse array of healthcare specialties in 
outpatient settings for people in the surrounding communities of 
Brooklyn. The organization provides practice management and 
administrative support services to individual physicians and/or small 
group practices throughout Brooklyn.

We are currently seeking a Nurse Practitioner – Care Coordinator for our 
Bensonhurst Family Practice. Along with serving as a patient advocate 
and navigator, you will play an integral role in in the implementation 
and development of a workflow for our Patient Centered Medical Home 
program (PCMH), a primary care model that provides coordinated and 
comprehensive care to patients that will help improve health outcomes.

*Responsibilities include:*
* Promoting coordination across the care continuum
* Overseeing staff and ensuring a team approach to patient care
* Providing ongoing education to staff and providers on PCMH
* Ensuring the tracking, reporting and data assimilation and analysis 
for all components of the PCMH program are satisfied
* Auditing EMH, patient satisfaction surveys, analysis and subsequent 
communication of findings to program physicians and staff
* Supervising daily activities of the Administrative Assistant, Office 
Assistant, Patient Assistants, and Clinical Assistants.

*Requirements include:*
* Graduate of an accredited school of Nursing with an active RN license 
Masters’ degree and/or certification as a Nurse Practitioner by New York 
State
* Current New York State License as an Adult, Acute Care or Family Nurse 
Practitioner
* Current American Nurses Credentialing Center (or equivalent) 
Certification as an Adult, Acute Care or Family Nurse Practitioner
* New York State Prescriptive Authority
* Current Registration with the Drug Enforcement Administration (DEA)
* Current Basic Cardiac Life Support and Advanced Cardiac Life Support 
Certification
* Knowledge of Word and Excel is required; EMR, Cerner or Athena 
strongly preferred
* Demonstrated skills in clinical quality improvement methodology and 
resource management
* Excellent organizational, interpersonal and communication skills
* Ability to handle issues that may arise between staff and/or 
physicians in a calm and professional manner
