PHYSICIAN ASSISTANT OR NURSE PRACTITIONER
OUTPATIENT INTERNAL MEDICINE
LOCATION: CONNECTICUT MEDICAL GROUP, HAMDEN, CONNECTICUT
OTHER OPPORTUNITEIS EXIST WITH NORTHEAST MEDICAL GROUP IN BRANFORD, 
GUILFORD, NEW HAVEN AND GREENWICH, CT

The primary function of the Advanced Practitioner (APRN/PA) is to 
provide comprehensive care to patients in collaboration with physicians 
and works with a designated supervising physician.

Assists physician(s) with patient care, has primary responsibility for 
medical record documentation and related paperwork and ordering of 
tests/procedures and sub-specialty consultations.

Identifies healthcare needs and assesses the status of patients and 
coordinates post-discharge follow-up care.

May assist with contacting patients' primary care physician. For new 
admissions, obtains comprehensive health histories, performs complete 
physical examinations, enters admission orders for appropriate 
laboratory and diagnostic imaging tests, and completes orders for 
therapy under the supervision of Physician(s).

Identifies self in EMR/MR as primary coverage person for assigned 
patients and updates pager list early in the day.

Collects all paperwork associated with the daily sign-out process and 
ensures that patient sign-outs are updated in EMR for assigned patients 
by shift end.

Identifies self to assigned patients and their families as the provider 
responsible for their care.

Serves as a liaison and interacts with ancillary services to ensure 
timely and comprehensive care for all patients.

_EDUCATION _
Graduation from an accredited Advanced Nursing or Physician Associate 
program required. Certified as a Physician Associate by the National 
Commission on Certification of Physician Associates. Both hold a valid 
Connecticut medical license, Connecticut Controlled Substance 
Registration and Federal Drug Enforcement Agency (DEA) Certification.

_EXPERIENCE_
One (1) to two (2) years caring for patients preferred.

Accountable to conduct accurate and appropriate evaluation, care, and 
coordination of care of patients.

_COMPLEXITY_ (describe planning, problem solving, decision making,

Evaluates, manages, and coordinates care of patients under the 
supervision or in collaboration with the Physician(s). May assist 
Physician(s) in all aspects of patient care, including interactions with 
other services involved in the care of patients, and possibly in the ED. 
Works with Management on special projects as assigned.

_LICENSURE/CERTIFICATION:_
Connecticut Licensed APRN or PA

_SPECIAL WORKING CONDITIONS AND PHYSICAL DEMAND:_
MMR is required. Varivax (chicken pox vaccine) or evidence of prior 
chicken pox is required.
Hepatitis B (or signed declination) is required for those with potential 
exposure to blood/body fluids.
Tdap and influenza vaccination are strongly encouraged.
