*Nurse Practitioner, Clinical Instructor, Internal Medicine, Endocrinology*

Stony Brook University School of Medicine of the State University of New 
York invites applications for the position of Nurse Practitioner in the 
division of Endocrinology.

*REQUIRED QUALIFICATIONS:*
Candidates must have a Master's degree in Nursing or completion of a 
course for Nurse Practitioners. Licensed as a Nurse Practitioner in NYS. 
Three years of experience in acute care. Eligible to sit for and must 
pass the CDE exam after one year of employment.

*PREFERRED QUALIFICATIONS:*
Experience with diabetes management and education in a hospital setting 
with knowledge of current clinical guidelines and standards from the 
ADA, AACE and AADE. Certified Diabetes Care and Education Specialist (CDE).

*Brief Description of Duties:*The Nurse Practitioner will see and treat 
patients with diabetes admitted to Stony Brook University Hospital and 
will demonstrate skills in collaboration, teamwork and problem solving 
to achieve desired goals.
* Independently assess and evaluate new and follow-up consults to the 
inpatient diabetes consultation service at Stony Brook University Hospital.
* Perform daily physical exams of patients with diabetes with special 
attention to body systems that may be affected by diabetes.
* Review 24 hour tests of blood glucose values and recommend daily 
orders for medication to primary clinical team.
* Educate patients and family members regarding diabetes self-management 
principles based on recognized curricula and national standards of care.
* Identify discharge needs at the point of initial patient contact and 
assist in the transition of care planning.
* Rotate through the established “call “schedule of the diabetes service 
and carry a paging device.
* Demonstrate skills in collaboration, teamwork and problem solving to 
achieve goals
* Knowledge of basic software packages and hospital information systems 
(EMR)
