*Nurse Practitioner – GENERAL MEDICINE*

Stony Brook Medicine’s Nurse Practitioners are valuable members of our 
team who provide clinical services to our patient population. We are 
seeking candidates to work full-time in our Primary Care Center to 
provide adult patient care under the supervision of the identified 
supervising physician. Qualified candidates will demonstrate superior 
patient care and possess outstanding communication skills while adhering 
to our high standard of excellence..

*Required Qualifications:*Master’s degree in nursing or completion of a 
course for Nurse Practitioner and a certification as a Nurse 
Practitioner in Adult or Geriatric care.

*Preferred Qualifications:*Additional years of Geriatric and Primary 
Care internal medicine experience, previous experience assisting with 
patient consultations. Motivation, dedication and flexibility are all 
desired qualities.

*Duties may include the following but are not limited to:*
* Assist with consultations and initial visits. Perform follow-up care, 
home visits and urgent care as assigned by division chief. Perform (or 
learn to perform) comprehensive geriatric assessment, including 
administration of functional screens, cognitive and depression screens 
and gait and balance assessments.
* Telephone patients to give lab results, arrange for prescription 
refills where appropriate. Write or call in new prescriptions for 
medications as permitted by practice agreement; triage phone calls when 
necessary.
* Coordinate follow-up care when instructed including consultation 
referral, ordering of diagnostic tests, arranging for home care 
services, ordering durable medical equipment, coordinating referrals to 
Social Work Nutrition, psychology and Rehabilitative services.
* Arrange for ER and inpatient admission/transfer in acutely ill patients.
* Perform relevant outpatient procedures (e.g., pelvic exams, cerumen 
and fecal disimpaction) as clinically indicated and as permitted by 
practice agreement.
* Provide nursing support to clinical research program and quality 
improvement initiatives within the Division of Geriatrics and General 
Internal Medicine.
* Perform other duties (including teaching) as assigned by Dr. Fields or 
designee.
