*Description*
The Division of Endocrinology & Metabolism in the Department of Medicine
is seeking an outpatient diabetes Nurse Practitioner who will work with
physicians and nurses. Successful incumbent will demonstrate excellent
communication skills as well as the ability to collaborate and resolve
issues to achieve desired goals.
* Adhere to standards of care in diabetes management, perform a physical
examination and make assessment and diagnosis based on this information
as well as initiate, review and revise treatment plans and record and
present data to the supervising physician.
* Assist physician with interventional treatments
* Respond to incoming phone messages from patients and make call backs
to patients who are reporting responses to their attending physician.
* Make referrals to other healthcare providers and/or consult the
physician for further definitive therapy, order and evaluate appropriate
non-invasive diagnostic tests and procedures.
* Instruct and counsel patients and family members regarding diabetes
self-management principles based on recognized curricula and national
standards of care.
* Other duties as assigned.
*Qualifications*
*REQUIRED QUALIFICATIONS:*
Candidates must have a Master's degree in Nursing or completion of a
course for Nurse Practitioners. Licensed as a Nurse Practitioner in NYS.
Eligible to sit for and must pass the Certified Diabetes Educator (CDE)
exam within two years of employment.
*PREFERRED QUALIFICATIONS:*
One year of post licensure experience as a Nurse Practitioner with
experience in diabetes management and/or Internal Medicine. Past
experience with patient education in an outpatient setting. Knowledge of
current clinical guidelines and standards from the American Diabetes
Association, American Association of Clinical Endocrinologists and
American Association of Diabetes Educators. Previous experience with EMR.
