*Palliative Care - Advance Practice Nurse/Adult Nurse Practitioner*

Park Nicollet Health Services is seeking an outstanding Palliative Care 
Advance
Practice Nurse or Adult Nurse Practitioner to join a thriving inpatient 
and outpatient
Palliative Care team. Join a dynamic and dedicated interdisciplinary 
team of 4
Palliative Care physicians, 2 social workers, a chaplain and a nurse 
coordinator in
providing great patient centered care to patients with advanced illness. 
In addition to
a mature inpatient consultative practice, our outpatient practice is 
fully integrated
within our innovative cancer center. This person will function as part 
of the Palliative
Care team to assess the needs of palliative care patients and families 
and assist in
the management of complicated symptoms and goals of care discussions. 
Clinical
Nurse Specialists or Physician Assistants will also be considered. 
Palliative care/
hospice education or experience preferred. Excellent teamwork, symptom
management and communication skills are a must.
