*Nurse Practitioner | Geriatric Medicine | Fort Collins, CO*

Are you patient-oriented and compassionate? Like to have clinical 
support from a
collaborating physician and administrative support? We are looking for a 
Nurse
Practitioner who enjoys serving the geriatric population. Our mission is 
to provide
high-quality medical care in a professional, caring, and personable 
atmosphere.
Our priority is the health and wellbeing of our patients in assisted 
living and skilled
care settings in northern Colorado.

*Compensation and Benefits:*

*Overview of Job Duties:*
Evaluate and treat patients in assisted living and skilled care settings.
Make diagnosis; refer patients to other healthcare providers.
Collaborate with PCP's and other Physicians.
Select, evaluate, interpret and follow up on all lab tests ordered.
Complete all health evaluations, summaries and chart notes in EMR system.
Maintain professionalism with patients, families, co-workers and 
facility staff.
Initiate and alternate appropriate medication within established protocols.
Take call on a rotating schedule.

*Qualifications:*
Must be Licensed as a Nurse Practitioner in Colorado.
Must hold DEA license/ Prescriptive authority for the State of Colorado.
2+ years of Practicing Experience as a Nurse Practitioner.

*Schedule:*
Full-time/Day time hours. 40-hour work-week Monday through Friday.

*Location:*
Fort Collins, Colorado – Service area in northern Colorado. Fort Collins is
considered to be one of the most beautiful areas of the county. Home to 
Colorado
State University with a 22,000 student enrollment. Fort Collins boasts a 
city
population of 140,000 offering multiple shopping, dining, and entertainment
options.
